To make your first appointment, please verify with your insurance that we are your Primary Care Provider (PCP) by following the steps below.
- Find your insurance name below and call the phone number that is listed.
- Tell your insurance that you would like to change your “Primary Care Provider” (also known as “PCP”).
- You will then be asked to select a new doctor or doctor’s office.
- If you would like to select Hurtt Family Health Clinic as your primary doctor’s office, tell the representative one of the “Provider ID” numbers below:

- Please note that it may take up to 30 days for the change to be effective.
