To make your first appointment, please verify with your insurance that we are your Primary Care Provider (PCP) by following the steps below.

  1. Find your insurance name below and call the phone number that is listed.
  2. Tell your insurance that you would like to change your “Primary Care Provider” (also known as “PCP”).
  3. You will then be asked to select a new doctor or doctor’s office.
    • If you would like to select Hurtt Family Health Clinic as your primary doctor’s office, tell the representative one of the “Provider ID” numbers below:

  4. Please note that it may take up to 30 days for the change to be effective.
